Proposal to Register a General Academic Program
Note: these forms are NOT intended for proposed teacher, educational leader, or pupil personnel services certification programs, or for programs that prepare individuals for professional licensure.
When completing these forms, please refer to theÌýgeneral academic guidance documentÌýfor more information.
Submission of Applications
1.ÌýÌýÌýÌý Create a single PDF document that includes the following completed forms:
- Appropriate application forms (see options under "Types of General Academic Proposals")
- Supplement(s) to the application (as required)
2.ÌýÌýÌýÌý Create a separate PDF document for any required syllabi (see Task 3 for syllabi requirements.)
3.ÌýÌýÌýÌý Attach the PDF documents to an e-mail.
4.ÌýÌýÌý Send the e-mail toÌýocuerevadmin@nysed.govÌýwith theÌý³§³Ü²úÂá±ð³¦³Ù:ÌýInstitution Name, Degree Award, and Program Title.ÌýExample: AAA College, New Program, Master of Science, English Literature

Distance ¶«¾©ÈÈColleges and universities located in or operating in New York State that offers degree or certificate programs in which a major portion (i.e., 50 percent or more) of the requirements can be completed through study delivered by distance education must have those programs registered in the distance education format. |

Master Plan AmendmentÌý(MPA)When an institution seeks to expand its academic mission (e.g., by offering a degree at a new level of study or in a new disciplinary area), it must submit a master plan amendment application for review and approval by the Board of Regents. |

External Review ReportAn external review of the proposed program is required if the program is a graduate degree program; or if the program's subject matter represents a new or emerging field; or if the external review is requested by the Department. |
